Please bring a valid photo ID and your insurance card so we can quickly register you and help you be seen by the provider. It is also very helpful to bring a written list of your medical history, including current medical conditions, medications, and allergies.
Completing as much of your registration material online before your visit can help speed up the check-in process. We also recommend reviewing your insurance benefits, deductible, and co-pay ahead of time to help avoid unexpected billing questions during or after your visit.
When you arrive, a staff member will greet you at the front desk. They will help you complete registration and make sure you are seen by the provider as quickly as possible.
